    The University of Louisville Trager Institute GWEP case study discusses integration of the 4Ms Framework into the EHR system through their innovative FlourishCare (FC) model.

    This initiative transformed their geriatrics primary care clinic into an interprofessional, age-friendly hub by embedding a holistic, biopsychosocial assessment directly into the EHR, enabling the creation of shared, goal-oriented care plans centered on what matters most. The case study details the development of specific workflows and custom functionalities within Epic that support interdisciplinary team communication and track improvements in mobility, mentation, and medication management.

  • Partnering with the Chickahominy Indian Tribe to Implement Age-Friendly Care – Virginia Geriatric Education Center GWEP

    The Virginia Geriatric Education Center (VGEC) GWEP is partnering with the Chickahominy Indian Tribe in Virginia to implement Age-Friendly Care for tribal elders. This case study shares the journey of embarking on relationship building and collaboration with the Chickahominy Indian Tribe, including establishing relationships with tribal leaders and staff, being present in the community for pow-wows and other events, conducting an age-friendly needs assessment with tribal elders, offering culturally adapted educational opportunities identified as important by the elders, and hiring a tribal citizen to join their GWEP team.

    The activities have generated positive impact and support coalition building with tribal nations. Currently, tribal leaders and elders are co-creating and delivering, with the VGEC GWEP, a video-based curriculum to train health care professionals and students about the history, culture and meaning of health and wellness in the tribal community.
