    159. Pellet Hormone Therapy: How Dosing Goes Wrong and How to Avoid It
    Oct 24 2026
Once a pellet goes in, it stays in. That's what makes the dose decision different from every other kind of hormone therapy. I'm not here to sell you on pellets. They aren't right for everyone, and the loudest criticism of them is fair. You can peel off a patch, but you can't take back an insertion. What that really means is that dosing has to be right the first time, and this week I'm giving you the three pearls I use to get there. We'll cover why starting low beats starting bold, the three levels that need testing before anyone puts a pellet in you, and what your hormone curve actually does over the weeks following an insertion. I'll also tell you what I was doing wrong in my first years of doing this, and why I threw out the dosing calculator my own brand provides. In this episode you'll learn… Why pellet therapy cannot be reversed once the insertion is done. Why underdosing is always safer than overdosing pellet therapy. What too much estradiol feels like after years without it. Which patient concerns should immediately signal a lower starting dose. Why a patch prescription does not equal optimized hormones. Which three hormone levels need testing before every insertion. What stacking is and how repeated insertions create it. Why skipping blood work to save money eventually backfires. When pellet hormone levels peak after an insertion. Why the same pellet dose produces blood levels over 40% apart between women. Why copying another woman's successful dose is a mistake. When to test after a first insertion or a dose change. What a booster pellet is and when it makes sense. 158. Hormone Pellet Therapy Benefits, Billing Insurance and More: The Inside Scoop
    Oct 16 2026
Ten hours of exercise a week, a clean gluten-free diet, and the scale went up ten pounds. That's what finally drove me to pellet therapy in 2018, and I haven't looked back since. But before I tell you that story, I need to deal with the question I know you're about to ask: can insurance cover this? Almost certainly not, and I want you to understand exactly why. I bill for it anyway, and I'll walk you through what that actually costs my practice, how long we wait to get paid, and what Medicare has done recently to make things even more difficult. If you've ever been annoyed at a bill for a pellet, this is the part nobody explains. In this episode you'll learn… Why hormone pellet therapy is almost never covered by insurance. Why an in-network pellet provider is nearly impossible to find. Why most practices add pellet therapy as a cash revenue stream. What stops hospital employed physicians from offering pellet therapy. What training, inventory, and sterile instruments cost a practice. How insurance reimbursement compares with what a pellet actually costs. What happens when a second pellet comes due before payment arrives. Why pre-authorization still does not guarantee any payment. Why Medicare stopped reimbursing pellet insertions over the past year. Why practices offering only one hormone option shortchange their patients. Why convenience drives most women toward choosing pellet therapy. What weight gain despite constant exercise revealed to me about missing hormones. How long optimized hormone levels were needed before my weight finally dropped. 157. Hormone Pellet Therapy for Menopause: The Advantages That Critics Ignore
    Oct 9 2026
Your heartbeat decides how much estradiol you get, and critics call that unpredictable. After weeks of discussing FDA approved alternatives to the patch, I'm turning to the one option that isn't FDA approved at all; the one that respected physicians attack hardest. Pellet therapy has been around since the 1930s, longer than anything else on the market. The loudest criticism is that pellet levels can't be relied on. I'll tell you what actually governs the release, why a faster heart rate giving you more hormone isn't the flaw everyone assumes, and what the patch does over two and a half days that nobody seems to mind. We all celebrated the insulin pump for doing something continuously that used to require daily shots. I'd like someone to explain why the same logic doesn't apply here. In this episode you'll learn… What the estradiol pellet delivers that no FDA approved product can. How a biodegradable pellet releases estradiol with every heartbeat. Why critics call pellet hormone levels unreliable and unpredictable. How exercise, stress, and anxiety speed up the rate of release. Why extra estradiol during stressful periods may be an advantage. Why patch levels return to baseline within two and a half days. What stacking means and why insertion timing matters so much. How quickly women tend to notice improvement after an insertion. What scar tissue and body fat do to the rate of release. How many pellet doses exist compared with five patch doses. Why pellet therapy predates every FDA approved estradiol option. Why the controversy traces back to excessive doses and early insertions. Why the patch holds one clear advantage over the pellet. 156. The Case Against the O-shot and Why I Completely Disagree
    Oct 2 2026
A respected gynecologist says the O-shot has no biological basis. I think she's wrong, and I can tell you exactly why. Dr. Jen Gunter has written twice that the O-shot is untested and built on a sketchy past. Both articles are linked below, and I'd encourage you to read them. Her central claim is that there's no damaged tissue down there to heal, so there's nothing for platelet rich plasma to do. I can name three circumstances that damage those exact tissues, and almost every woman listening has lived through at least one. Where I don't disagree with her is the missing research. So I'll explain why these studies are so hard to run, what the few that exist actually found, and the one group of women I usually talk out of this procedure entirely. In this episode you'll learn… Why critics argue there's no damaged tissue down there to repair. How many women sustain tissue trauma during a first vaginal birth. Why the clitoris can be injured during delivery without anyone noticing. What portion of women develop skin conditions affecting the vaginal opening. Why the vulva has been called the forgotten pelvic organ. How menopause damages the same tissues the injection targets. Why comparing the injection to surgery misses what actually happens. What PRP did for women left with painful scars after surgery. Which once standard procedures were abandoned after proper trials. Why a convincing placebo for this procedure is nearly impossible. Why studies rarely follow the actual trademarked injection protocol. What a review of the existing research found about incontinence. Why age changes how well the injection is likely to work. 155. Better Orgasms and Less Incontinence From a Single Injection? Benefits of the O-shot
    Sep 25 2026
There's an injection that improves orgasms and urinary leaking at the same time, and almost no woman has heard of it. The O-shot involves no estrogen and no hormones. It's made from your own blood, drawn from your arm, and spun down before it goes anywhere. The needle is the same one we use for Botox. It was designed around sexual function, and it delivers on that. But something else kept happening to the women who had it done, something nobody was aiming for, and that's become one of the main reasons I've come to recommend it. In this episode you'll learn… What the O-shot injects and where the two injections go. Why the injection hurts far less than most women expect. What platelet rich plasma is and when the term first appeared. How researchers discovered that platelets could stimulate tissue repair. What separates platelet rich plasma from platelet rich fibrin. Why PRP already appears in microneedling, hair restoration, and orthopedics. What gynecologists have been treating with PRP beyond sexual function. Why insurance companies label these gynecological applications as experimental. What a review of the published PRP research actually found. Who coined the term O-shot and trains providers to perform it. What a provider should feel immediately after injecting the G-spot. What the Skene's glands are and where they sit. How those glands help stop bacteria from reaching the urethra. Why nourishing erectile tissue changes arousal and orgasm quality. How long improvements in urinary leaking tend to last. • 154. Vaginal Estrogen Options for Menopause Compared: FDA Approved and Compounded

    154. Vaginal Estrogen Options for Menopause Compared: FDA Approved and Compounded
    Sep 18 2026
Every vaginal estrogen product treats the inside. Only one of them treats the outside. Last week, I made the case that vaginal estrogen does not stay where you put it. This week, I go through what is actually available, and what each option can and cannot do for you. We cover the creams, the inserts, the ring, and the one FDA approved product that contains no estrogen at all. One of these treats something none of the others can touch, which is why I keep prescribing it despite the mess. I also get into compounded formulations and read the ingredient list on a common estradiol cream so you can hear what you are really putting in your body. In this episode you'll learn… Which three vaginal estrogen options carry FDA approval in the United States. Why the cream treats external vulvar symptoms the other options cannot. What makes cream dosing less precise than inserts and rings. Why conjugated equine estrogen cream offers no advantage over estradiol. How the softgel insert helps women who struggle with applicators. Why stopping inserts for weeks can undo months of symptom control. What Estring releases daily and how long it stays in place. How four applications a year compare with over one hundred. Why the ring suits women who cannot remember twice weekly treatment. What conditions make the ring unsafe to insert at all. What Intrarosa offers women who cannot use estrogen at all. Why daily use turns a local vaginal treatment into whole body therapy. How compounded products are regulated despite lacking FDA approval. What the ingredient list on a common estradiol cream actually contains. Why estriol has recently become popular in the aesthetic world. 153. The Genitourinary Syndrome of Menopause and The 2 Safeguards All Users of Vaginal Estrogen Need
    Sep 11 2026
Women have been told for decades that vaginal estrogen stays where you put it, which means no progesterone and nothing to monitor. I do not believe that is true, and this week I lay out why. We start with what genitourinary syndrome of menopause, or GSM, actually looks like, including the urinary half of it that almost nobody connects back to hormones. Then I make my case for what happens to the estrogen after it leaves the vaginal tissue, and what two safeguards I think every woman using it should have. I also share an ultrasound finding that changed how I think about yearly screening, in a patient who was not using hormones at all. In this episode you'll learn… What separates local hormone therapy from whole body hormone therapy. What genitourinary syndrome of menopause (GSM) covers beyond vaginal dryness. Why dryness comes from estrogen loss rather than from aging. How the tissues change without the blood flow estrogen provides. Why painful sex often goes unmentioned until a relationship is already threatened. Why recurrent urinary infections after 50 are a hallmark of GSM. Why medical training has left doctors unprepared to recognize GSM. What drives the bladder spasms behind urgency and frequency. How pelvic floor treatment compares with thousands of Kegel contractions. What common overactive bladder medications carry as long term risks. Why the vagina has long been used to deliver medication into the body. How absorption from vaginal estrogen varies enormously between women. Why some women need progesterone despite using only vaginal estrogen. Why blood testing will almost never detect vaginal estradiol. Why whole body hormone therapy does not always remove the need for vaginal estrogen. 152. Can Estradiol Spray or Gel Replace Your Sold-Out Patches?
    Sep 4 2026
Two of the best alternative options for estradiol come with instructions almost nobody reads. The patch is not the only thing running short. This week, I continue the series with two more alternatives: one that almost no physician thinks to offer, and one that has quietly become the most requested. We look at transdermal spray and estradiol gel, how each one is applied, and what those oddly specific application rules exist to prevent. The number printed on the label turns out to have very little to do with how much estradiol actually reaches your bloodstream. I also explain why one of these products has never once shown me what it's supposed to in my own patients' lab results. In this episode you'll learn… Why transdermal estradiol spray remains unknown to most prescribing physicians. How additional daily sprays must be placed to avoid overlapping skin. Why three or four sprays daily create hormone surges and crashes. What those surges can trigger beyond the symptoms being treated. How estradiol transfers to children, pets, and partners after application. Why the spray delivers far less estradiol than each dose suggests. What FSH testing reveals about how well the spray absorbs. Why the spray carries narrower FDA approval than other estradiol options. What the spray costs monthly with and without discount programs. Why estradiol gel sits at the top of many plan B lists. How three FDA approved gel products differ in delivery and dosing. Why single dose packets make adjusting the dose easier than pumps. What metered dose pumps require before the very first use. Why only a small fraction of applied gel reaches circulation. What can happen when children are unknowingly exposed to estradiol.
