    In this episode I’m breaking down the five most common “fertility adjuncts” men use alongside TRT: enclomiphene, hCG, hMG, kisspeptin, and gonadorelin.


    Taylor and I are in the middle of our own family-planning season, so I’ve been going deep on what actually preserves fertility on testosterone, what helps you actively move sperm parameters when you’re trying to conceive, and what’s mostly just a cheaper workaround that gets overhyped.


    We’ll start with the core problem: TRT suppresses LH and FSH, which over time can reduce intratesticular testosterone, sperm output, and testicular fullness. Then I’ll compare each option in plain English—how it works, who it’s best for, and where it falls short.


    You’ll hear why I view hCG as the backbone for most men, when hMG becomes the “needle mover” for conception, where enclomiphene fits (and why I don’t love it as a decades-long solution), why kisspeptin is more of a libido layer than a true fertility replacement, and why gonadorelin is mainly a budget play for guys who are done having kids.


    Big takeaway: TRT doesn’t automatically mean sacrificing fertility—if you use the right strategy.

    In this episode, I finally dive deep into a peptide I’ve wanted to talk about for a long time: P21.


    P21 is what I would call a true brain health peptide. It’s not a nootropic in the classic “stimulant” sense, and it’s definitely not a limitless-pill type compound. Instead, it works upstream, supporting the brain by upregulating BDNF, enhancing neurogenesis, improving synaptic connectivity, and reducing neuroinflammation.


    I first heard about P21 years ago through parents of neurodivergent children who were experimenting with intranasal use and seeing meaningful changes. That sent me down the research rabbit hole. Since then, I’ve used it personally, worked with others using it, and spent a lot of time reviewing the animal data behind it.


    In this episode, I break down what P21 actually is, how it works mechanistically, why it’s often compared to Cerebrolysin (and why it’s very different), and where I see its real-world use cases. We talk memory, neurogenesis, Alzheimer’s and Down syndrome animal models, traumatic brain injury, vision effects, dosing strategies, safety data, and how I personally cycle it.


    This isn’t hype. There are no human trials yet. But the preclinical data is compelling, and in a world where sourcing Cerebrolysin is harder than ever, P21 stands out as a lighter, more accessible tool for long-term brain health.

We’re talking sildenafil for longevity, healthspan, and performance. The “little blue pill” has a reputation as an erectile dysfunction drug, but when you actually look at the mechanism and the data, it’s doing way more than that.I’ve been a proponent of low-dose Cialis for a long time, and I still am. But lately I’ve been experimenting with rotating between Cialis and Viagra because I notice I can build a tolerance to Cialis over time. It still works, but I don’t feel the performance benefits as strongly unless I push the dose higher. What I’ve found is that going four weeks on low-dose Cialis, then four weeks on low-dose Viagra, seems to keep me more responsive overall while still giving me the benefits that matter for longevity.In this episode, I break down the real mechanism. Sildenafil is a PDE5 inhibitor that amplifies nitric oxide signaling and raises cGMP throughout the body. That means systemic vasodilation, improved endothelial function, better blood flow to organs, and anti-thrombotic effects through reduced platelet activation. And the downstream effects get even more interesting. We’re talking mitochondrial signaling through PKG, potential ischemic preconditioning effects, PGC-1α and mitochondrial biogenesis, AMPK activation, and dampening inflammatory pathways like NF-kB. This is why I think sildenafil deserves a seat at the longevity table, not just a spot in the performance category.Then we get into the human data and what I think is most compelling. Large datasets show associations with lower rates of cardiovascular events and lower all-cause mortality in PDE5 inhibitor users. We talk heart attacks, strokes, endothelial repair, and why this class may act like an exercise mimetic for certain people by improving hemodynamics and oxygen delivery. We also cover pulmonary benefits, including its FDA-approved use for pulmonary arterial hypertension, and why that matters for aging lungs, sleep apnea-related pulmonary issues, and even high-altitude physiology.The brain section is the one that makes most people stop and rethink everything. There are observational datasets suggesting meaningfully lower Alzheimer’s and dementia incidence in sildenafil users, plus plausible mechanisms tied to cerebral blood flow, synaptic plasticity, inflammation reduction, and mitochondrial support inside the brain. I’m not saying it’s a magic bullet. I am saying it’s one of the more interesting repurposing conversations in longevity right now.
