A Dublin lab knew anti-D batches were contaminated in 1977, but injections continued-what happened next?

The Injection They Knew Was Poisonous - Mothers Left to Die

In this episode, we follow the sequence of decisions, missed recalls, and the emergence of a group of women who refused to stay silent about infections from anti-D blood product. What choices did institutions make once contamination was known, and how did women like Brigid McCole respond?

Person: Brigid Ellen McCole
Date: 25 July 1977 (instruction to stop using Patient X’s plasma)
Location: Blood Transfusion Service Board, Dublin; maternity wards across Ireland
Event: Contaminated anti-D batches delivered and administered
Group: Positive Action (formed May 1994)

- A single instruction from a BTSB biochemist ordered that Patient X’s plasma be stopped on 25 July 1977, but contaminated anti-D batches were already packed and sent to maternity wards.
- Anti-D was a routine postnatal injection given across Ireland since 1969; in 1977 it was trusted and administrated without suspicion.
- A 16 December 1991 fax from Dr. Jeremy Garson at Middlesex Hospital reported Patient X-linked samples tested positive for hepatitis C, prompting questions that went unanswered by the BTSB.
- Internal BTSB review in 1994 linked new hepatitis C positive donors-mostly women in a narrow age range with similar reproductive histories-to the 1976-77 anti-D batches and notified the Health Minister on 17 February 1994.
- Women infected sought each other out after the BTSB refused to connect them; by May 1994 twenty-five women met in Dublin and formed Positive Action to organize and pursue accountability.
