Supported Decision-Making (SDM) is an alternative to guardianship that allows individuals with disabilities to retain their legal rights and make their own life choices with help from a chosen team of trusted family members, friends, or professionals.

Jonathan Martinis is the founder and principal of LightPath Advocacy. LightPath provides legal, education, and consulting services across the United States designed to ensure that people with disabilities can access the supports and services they need to live their best lives.
Anita Raymond, LISW, CMC is the Director of the Center for Excellence in Supported Decision Making and Care Management & Consultation programs at VOA MN & WI. CESDM provides direct social work seeking the least restrictive, most person-centered decision making arrangements, and operates Minnesota’s Guardianship Information Line, responding to over 1200 inquiries per year from people at risk of or subject to guardianship, their families, communities, and the professionals who work with them.
