Optimize the Patient for the Treatment
Ventricular fibrillation is a shockable rhythm—but does that mean the best treatment is always to shock simply because the clock says it's time?
In this episode of the ACR Moonshot Podcast, Billy Croft and Joe Powell challenge the traditional time-based approach to ventricular fibrillation and ask a different question:
Is the patient physiologically ready for the treatment we're about to deliver?
Billy and Joe explore what is actually happening during VF and why successful defibrillation may require more than simply identifying a shockable rhythm and charging the monitor.
The discussion looks at the importance of high-quality CPR, maximizing chest compression fraction, reversing hypoxia and acidosis, restoring the conditions necessary for the myocardium to respond, and understanding the difference between a witnessed arrest and a patient who may have already been in cardiac arrest for several minutes.
They also discuss refractory ventricular fibrillation, medication strategies, and the role of dual sequential defibrillation when traditional approaches aren't working.
The goal isn't to delay defibrillation.
The goal is to understand when the patient needs immediate defibrillation—and when the physiology may need to be optimized first.
Because cardiac arrest isn't treated by a stopwatch.
Don't shock the clock. Optimize the patient for the treatment.
Protect the Gradient.
