The Restorative Care Pathway (RCP) is one of the most exciting opportunities within Australia's new Support at Home program—but what is it, and how can it help older Australians maintain their independence?

In this episode, Scott Lynch and Jacinta Cunningham explore how the Restorative Care Pathway works, who it's designed for, and why the move from an 8-week to a 16-week program is a significant step forward for achieving meaningful clinical outcomes. They also discuss person-centred goal setting, the role of allied health, and how restorative care can help older Australians regain confidence, improve function, and continue living well at home.

The integrated assessment tool (IAT) has raised many questions among providers and clients alike. Why is this shift happening now, and what does it mean for the future of support at home?

Navigating these changes may feel daunting, but focusing on quality improvements can make a huge difference.

In this episode, we'll unpack the key insights shared by Scott Lynch, founding director of Community Therapy, during his recent discussion with Jacinta Cunningham on the Community Therapy podcast.

Key Takeaways for Providers and Clients

• Transparency is Key: Understanding how the integrated assessment tool works will empower clients and providers alike.
• Honest Communication: Clients should present their true needs during assessments to ensure adequate support.
• Adaptation to New Processes: Providers must navigate new claiming rules while maintaining a focus on quality service delivery.

In our latest podcast episode, Scott and Erin discuss how inclusive practice isn't about having all the right answers — it's about listening.

As healthcare professionals, one of the most powerful tools we have is the ability to create space for people to tell their story.

By approaching care with curiosity instead of assumptions, and by focusing on what matters most to the individual, we can help people feel seen, heard, and supported.

Whether supporting someone from the LGBTQIA+ community, a First Nations background, a culturally diverse community, someone living with mental health challenges or anyone with a unique lived experience, the foundations of great care remain the same:

✨ Listen without assumptions.
✨ Lead with curiosity, not judgment.
✨ Create a safe space for people to share their story.
✨ Focus on their goals, values, and experiences.

Because at the heart of person-centred care is simply seeing the person in front of you.

Key Takeaways
Creating an inclusive healthcare environment is an ongoing journey that requires commitment, awareness, and education. As we celebrate Pride Month, let’s focus on:
- Using inclusive language and practices that respect individual identities.
- Building our toolkit for sensitive practice and encouraging open communication.
- Seeking continuous feedback to improve our approaches and ensure they meet the needs of our diverse patient population.

By committing to these practices, we can create a supportive environment for all patients and honor the spirit of Pride Month every day of the year.

In this episode, Scott Lynch (Community Therapy's Founder and Managing Director) and Jacinta Cunningham (AHPRA registered Occupational Therapist, and Learning & Development Lead at Community Therapy) discuss the recent reforms in Australia's aged care funding, focusing on the Support at Home and Home Modifications schemes. They explore challenges, opportunities, and future directions for allied health professionals and care partners navigating this complex landscape.

The Support at Home program represents a significant shift in aged care funding, especially with the introduction of the AT and home modifications budget. Unlike previous systems that required older adults to save toward equipment or modifications, this new scheme provides a dedicated budget, streamlining access to essential supports.
